Ontario Health Premium explained (2026)

The Ontario Health Premium is an extra amount added to Ontario income tax for anyone with taxable income over $20,000. It ranges from $0 to $900 a year and is deducted from your paycheque.

Who pays it

Every Ontario resident with taxable income above $20,000 pays it as part of provincial income tax. It's withheld from your paycheque along with your other taxes. It doesn't change your OHIP coverage; everyone is covered whether they pay it or not.

The formula

  • Up to $20,000: $0
  • $20,000 to $36,000: 6% of income over $20,000, maximum $300
  • $36,000 to $48,000: $300 plus 6% over $36,000, maximum $450
  • $48,000 to $72,000: $450 plus 25% over $48,000, maximum $600
  • $72,000 to $200,000: $600 plus 25% over $72,000, maximum $750
  • Over $200,000: $750 plus 25% over $200,000, maximum $900

Because it rises in quick steps, the premium is fixed at $600 for most incomes between $48,600 and $72,000, and $750 for most incomes between $72,600 and $200,000.

How to lower it

The premium is based on taxable income, so RRSP contributions, the enhanced part of CPP and CPP2 reduce it. See the full effect on your pay with the Ontario income tax calculator, which includes the premium automatically.

Frequently asked questions

Do I have to pay the Ontario Health Premium?

Yes, if you live in Ontario on December 31 and your taxable income is over $20,000. It is collected through payroll deductions and your tax return.

What is the maximum Ontario Health Premium?

$900 a year, for taxable income above $200,600.

Is the Ontario Health Premium the same as OHIP?

No. OHIP is the health insurance plan and is free for eligible residents. The premium is a tax that helps fund health care; it does not affect your coverage.
